Dear Parents and Guardians,

Our district flu shot clinic is scheduled for Wednesday, October 14th.  Consent forms are going home this week and are due back by Friday, October 9th. There are 2 consent forms that need to be returned-the Health department form and the Pea Ridge Schools release form.  Please reach out to Amy Jasay with any questions.
